Transcript: I'd like to demonstrate placement of the incision for a facelift, in my practice. The incision starts just behind the hairline here, it goes underneath the sideburn, right here. In front of the ear, here. On the inside of the tragus, right here so it's on the inside of the ear. Comes out right there, drops right in the very first crease around the earlobe. The incision then is completely located behind the ear, comes across here at the hairline, down, the incision follows the hairline down this way, following the hair follicle. Where the hair follicles grow in a more posterior direction, the incision follows that so it's usually about an inch down here and about an inch back this way. That way, when the extra skin is brought back from the neck to this location, the bald skin is removed, not the hair-bearing skin. And the same way here, when the jowls get lifted up here, the non-hair-bearing skin, or the bald skin is removed not the hair skin. What we're trying to do here is prevent raising the hairline. If you raise a woman's hairline, then she looks older, or windblown, or wind tunneled. So, it's critically important not to raise the hairline and that's why we place the incisions in this location. In addition, there's also a one inch right underneath here in the chin. And that's where we gain access to the neck. That's where we tighten the neck muscles, remove any fatty deposits in the neck, and then we're tightening loose facial and neck skin. The muscles are tightened in three locations, here in the central part of the neck, here at the jawline, and then the gristle layer is tightened along in this area and along in here, and the jowls get lifted and the tension on the incision is on the muscle and the gristle layer underneath, not on the skin. And that will prevent a wind-tunneled appearance. And it will give a very natural appearance to the lower face, neck lift procedure.
