55歲男性在外院大腸鏡檢查升結腸發現3cm LST，檢查醫師懷疑惡性腫瘤建議手術所以做了tattoo與切片檢查，但是切片顯示為良性腺瘤，所以病人來本院諮詢ESD的可能性． 因為已經打過tattoo ，所以預期中fibrosis 會比較嚴重且粘膜下都是黑色染劑，所以為了能夠更好鑽入粘膜下組織，distal attachment 改為使用ST hood 前端比 狹窄的cap能夠更有效率的進入粘膜下進行剝離． A 55-year-old male underwent colonoscopy at an outside hospital, where a 3 cm LST was found in the ascending colon. The examining physician suspected malignancy and recommended surgery, performing both a tattoo and biopsy. However, the biopsy result revealed a benign adenoma, so the patient came to our hospital for consultation regarding the possibility of ESD. Given that tattooing had already been performed, significant submucosal fibrosis and the presence of black dye throughout the submucosal layer were anticipated. To facilitate better entry into the submucosal space, we opted to use an ST hood instead of a conventional narrow cap, as its tapered design allows for more efficient access and dissection in fibrotic areas.
