I'm a facial plastic surgeon and rhinoplasty specialist at Penn Medicine Becker ENT, and this is two minute consults. One of the more common questions we get is how do we fix a big or a bulbous tip? In the past, people would be doing surgery to reduce the size of someone's tip. This is all based on the size and shape of someone's tip cartilages. So our tip cartilages come from around here and they come out to about here. If someone's tip cartilages have a shape that looks like this, then what people used to do during rhinoplasty is they would simply make them smaller. They would cut out excess cartilage up in this area to make the tip look smaller, and then tighten it with sutures. It can work in some percentage of the cases. However, there is a certain percentage of cases where it won't work as well. And more importantly, it can weaken the sidewalls of your nose and impact breathing. And that's something that is really critical and really something important to me. Because if some nose looks really good after rhinoplasty but it doesn't breathe very well, then I consider that a failure. In modern rhinoplasty, what we do here in my practice is that instead of cutting out cartilage and weakening the nose, we actually try to reshape things. And so what we do is we use a combination of sutures, we use a combination of cartilage grafting to really change the shape of someone's tip cartilage, to not only change it in a positive way for their aesthetics, but also ensure that we're improving and maintaining their breathing so the function of their nose is paramount.
