1. On 10th June 1990, British Airways Flight 5390 experienced an explosive decompression when a windscreen panel became improperly installed and separated from its frame. The incident occurred while the BAC One-Eleven 528FL was flying over Didcot, Oxfordshire, resulting in the captain's partial ejection from the aircraft. 2. Aloha Airlines Flight 243 April 28, 1988. As Aloha Airlines Flight 243 leveled at level 240 on an inter-island flight from Hilo to Honolulu, an 18-foot-long section of the upper fuselage suddenly departed the airplane, sweeping a flight attendant overboard. 3. Alaska Airlines Flight 261. On January 31, 2000, at about 1621 Pacific Standard Time, Alaska Airlines Flight 261, a McDonnell Douglas MD-83, N963AS, crashed into the Pacific Ocean about 2.7 miles north of Anacapa Island, California. All 88 people on board were killed and the airplane was destroyed on impact. 4. Japan Air Lines Flight 123. On August 12, 1985, the Boeing 747 operating the service suffered a severe structural failure and decompression 12 minutes into the flight 5. Tenerife airport disaster, KLM flight 4805, and Pan Am flight 1736 March 27, 1977. The collision of KLM 4805 and Pan Am 1736 caused a fire that killed everyone on board KLM 4805 and most of the other flight's occupants.
