Caregiver Routine Script Step 1: Filling the Bathtub While filling the bathtub, retrieve the large bottle labeled “body wash” from the back of the toilet. It is important to use that because “shampoo” will irritate the patient’s skin. Remove the cap and pour two capfuls of body wash into the bathtub. Reseal the bottle and return it to where you found it. Step 2: Chair Wipe Down Required: Wipe down the chair handles and the arms of the chair and the brakes both handles and in contact with a wheel – with one Pampers Wipe (same one for both). Step 3: Sheet Removal Inform the client that you’ll be removing their dirty sheets. (Note: sheets are changed every workday due to a skin rash condition.) Remove the top sheet, bottom sheet, and pillowcase. Place the pillow on top of the white Sterilite container on the silver fridge to keep it off the mattress. Sort and place linens into the correct hampers: Sheets → Sheets bin Pillowcase → Towels/washcloths bin (due to size) Step 5: Pre-Shower Cleanup & Transfer Pre-Wipe Down (Very Important) Before removing the shirt, use Pampers Wipes to wipe the front and backside of the client. Use four wipes for the front, and two wipes for the back. Use each wipe individually, and open it out fully, in your gloved hand Make sure to clean: Inside of both thighs Scrotum Perineum (area further back from scrotum) Backside (firm but gentle pressure) Diaper Removal Let the client know the bedding is done and ask if they're ready. If yes, put on gloves. Retrieve the black scissors from the toaster inside the bathroom cabinet. Cut the diaper from the outer thigh side while holding the inner side to avoid spills. Have the client hold the side rail (left rail is more stable). Discard the diaper in trash. Ask the client if you may press down the trash if it's full. Post-Diaper Cleanup Remove gloves, wash and dry hands, put on new gloves. Set out 6 Pampers Wipes (7 if no bath). Remove shirt: arms first, then head, avoiding the eyes. Discard the shirt in a dirty linens bin. Transfer to Tub Tell the client you’ll help them up by the waist. Help them place their right foot, then left, into the tub (check water temp first). Support waist and back as they sit. Place chuck pads: one in front of the tub, one in front of the sink. Step 6: Bath/Shower Cycle Bathing Sections Client will choose the order. Ask: Midsection? Hair? Arms? Use a different-colored washcloth for each section: Midsection/backside Arms/underarms Hair For hair: Use green shampoo Be gentle (sensitive scalp) Wash beard with minimal soap (avoid mouth area) For body: Use white Dove body wash Clean arms (shoulder to wrist, not hands) Rinse washcloth after underarms before doing chest/stomach Rub stomach in gentle circles (aids digestion) Rinsing Cycle Support the client to sit up if lying down. Turn on the tub faucet first, to around 3pm on a clock dial, confirm the temperature. Decisively Switch to shower mode, rinse all body parts except face (he does face). Communicate what part you're rinsing (left/right arm, chest, etc.) Post-Rinse Drying with Chuck Pads Face: Give him a chuck pad. Hair: Dry gently. Tub Setup: One chuck behind back (against wall) One behind body (against tub) One vertically for back support One for left tub side (white side up unless soaking up mess) Help him sit back and lay down (support back/waist) Dry sections in order: Arms, hands Chest/stomach Legs Midsection (last) Discard each chuck after use Transfer to Wheelchair Push the locked wheelchair in front of the tub. Pivot client to edge of the bathtub. Support and pull the client onto the chair using the blue chuck from before. Unlock the chair and roll into the room. Step 7: Post-Shower Oiling & Dressing Oiling Uncap baby oil (set cap on chair) Oil in this order: Arms/back of hands Chest/stomach Right thigh & shin (rash area) Midsection (grab enough oil first!) Cap bottle with clean hand. Discard gloves, wash and dry hands, put on new gloves. Dressing Get white shirt from the top shelf of white fridge. Put on shirt: arms first, then gently over head (avoid eyes). Adjust front and back as needed (client will instruct). Diaper Application Get a diaper from the sink area or hallway box. Make sure blue/green lines face forward. Have the client place legs in leg holes. While client holds armrests, quickly: Remove blue chuck (from tub, if showered) Pull up diaper in front only (don’t touch backside) Client may ask you to adjust before sitting. Discard gloves, wash and dry hands. Wheel client to computer.
